SPRING FESTIVAL AND PETTING ZOO LIABILITY WAIVER TERMS

CHURCH ACTIVITY/PHOTOGRAPHY WAIVER The undersigned parent/legal guardian hereby grants permission to Madi son Christian Church, Inc. (‘MCC”), for the child(ren) in my custody, listed on the reverse side of this form, to participate in today’s event. Should the listed child(ren) require immediate or emergency medical care while engaged in an activity sponsored by the Church, in my absence, I hereby grant the Church authority to release my child for medical treatment to such medical personnel as the Church determines appropriate under the circumstances. In consideration for the privilege of allowing my child to participate in the activity, I agree to release and hold harmless the Church, its officers and agents, from any liability to or responsibility for bodily injury, damage or illness to the listed child(ren) while participating in the day’s activities sponsored by the Church. Further, I agree to indemnify and hold harmless the Church, its officers and agents with respect to any claim asserted by or on behalf of the listed child(ren) as a result of bodily injury, illness, or dam age. I hereby authorize Madison Christian Church, Inc. (‘MCC”) to publish the photographs taken of me and/or the listed minor children, for use in the MCC’s printed publications, website, and other marketing materials. I release MCC from any expectation of confidentiality for the listed minor children and myself and attest that I am the parent or legal guardian of these children and that I have the authority to authorize MCC to use their photographs. I acknowledge that since participation in publications and websites produced by MCC is voluntary, neither the listed minor children nor I will receive financial compensation. I further agree that participation in any publication and website produced by MCC confers no rights of ownership whatsoever. I release MCC, its officers, trustees, and its employees from liability for any claims by me or any third party in connection with my participation or the participation of the listed minor children.

I,understand and agree to the following terms and conditions in consideration for being allowed to participate in the petting zoo activities provided by Apalachee Farms Mobile Petting Zoo:
1. I acknowledge that there are inherent risks involved with interacting with animals at the petting zoo, including but not limited to bites, scratches, diseases, and allergic reactions.
2. I agree to follow all instructions and guidelines provided by the staff at the petting zoo to ensure my safety and the safety of the animals.
3. I understand that Apalachee Farms Mobile Petting Zoo is not responsible for any injuries or damage that may occur because of my participation in the petting zoo activities.
4. I agree to release and hold harmless Apalachee Farms Mobile Petting Zoo, its employees, and representatives from all claims, including but not limited to personal injury, property damage, or loss that may arise from my participation in the petting zoo.
5. I acknowledge that I am participating in the petting zoo activities voluntarily and of my own free will.
*ALSO, for the SAFETY of PEOPLE and our ANIMALS, we ask that everyone, especially children, stay back when we are setting up or cleaning up the petting zoo. Please do not approach the set up until it is time to begin. As well as exiting the petting zoo area when the party/event time is over. Animals will be
brought in and out of the area which can be very stressful for the animals if they are spooked. Thank you for your cooperation*
